Are you a caregiver or a guardian?
To gain access to the Member Portal, you must be at least 18 years old. Contact our Member Services team, who will verify your legal guardianship status.
You will be able to log in to the Member Portal 24 hours after confirming legal guardianship.
No HIPAA or AOR forms will be accepted as proof of guardianship.
