Participates in the member’s care plan meetings
Conducts routine monthly rounding visits with members, focusing on preventive care and wellness
Performs Transition of Care Management visits
Ensures medications work together safely and eliminates any overlap
Care Ally
Communicates with the member’s care providers to ensure seamless care services
Participates in creating care plans and explains the plan to members
Helps coordinate specialist appointments, including referrals
Ensures smooth and safe transitions from hospital to home
Offers timely support for members’ questions about plan benefits
Conducts an annual health assessment to understand changing care needs
